Our Work
Building Understanding, Inclusion and Dementia-Friendly Communities Across Hertfordshire
OUR WORK.
Our Six Areas of Community Support
Dementia Outreach CIC works to build more inclusive and dementia-friendly communities across Hertfordshire. Our work focuses on six key areas, bringing together awareness, social connection, family support, wellbeing activities, community outreach and partnerships to improve the everyday lives of people affected by dementia.
Dementia Awareness & Education
We promote greater understanding of dementia through community awareness campaigns, educational activities and accessible information. Our aim is to challenge misconceptions, reduce stigma and help create more inclusive, dementia-friendly communities across Hertfordshire.
Social Inclusion & Connection
We create opportunities for people living with dementia to build friendships, enjoy meaningful conversations and participate in inclusive social activities. Through community engagement, we aim to reduce loneliness, strengthen connections and promote a sense of belonging across Hertfordshire.
Family & Carer Support
We aim to support families and unpaid carers affected by dementia through practical information, helpful resources, signposting to local services and opportunities to connect with others. Our focus is on reducing isolation and helping families feel informed, understood and supported throughout their dementia journey.
Activities & Wellbeing
We promote physical, mental and emotional wellbeing through dementia-friendly walks, gentle outdoor activities, creative sessions and opportunities for meaningful participation. Our activities aim to encourage independence, confidence, enjoyment and connection with the wider community.
Community Outreach & Engagement
We work with local communities to raise dementia awareness, encourage participation and reach people who may be isolated or experiencing barriers to support. Through outreach activities and community engagement, we aim to build stronger, more welcoming and dementia-friendly neighbourhoods across Hertfordshire.
Partnerships & Collaboration
We work to build partnerships with local organisations, businesses, community groups, healthcare professionals and volunteers. By sharing knowledge, resources and opportunities, we aim to strengthen community support and help make Hertfordshire a more inclusive and dementia-friendly place for everyone.
Together, We Can Build Dementia-Friendly Communities
Everyone has a role to play in creating communities where people living with dementia feel valued, included and supported. Whether you are a family member, volunteer, local organisation or business, your involvement can help us raise awareness, reduce isolation and make a meaningful difference across Hertfordshire.
How We Work
How We Connect and Support Our Community
At Dementia Outreach CIC, we believe meaningful community support begins with listening, understanding and working together. Our approach focuses on identifying local needs, creating inclusive opportunities, encouraging participation and building lasting partnerships that benefit people living with dementia, their families and carers.
Listen & Understand
We listen to people living with dementia, their families and carers to understand their experiences, challenges and priorities. Their voices help shape our community activities and outreach programmes.
Plan Together
We work with community members, families, volunteers and local organisations to plan meaningful, accessible and dementia-friendly activities that reflect people's interests, needs and abilities.
Engage & Connect
We bring people together through dementia-friendly activities, social gatherings, community walks and outreach programmes. Our aim is to encourage participation, build friendships and create a sense of belonging.
Review & Improve
We welcome feedback from participants, families, carers and community partners to understand what works well and where improvements are needed. This helps us develop activities that remain inclusive, meaningful and responsive to our community's changing needs.
A dementia-friendly community is one where people living with dementia feel understood, respected and included. Through awareness, education, meaningful activities and local partnerships, Dementia Outreach CIC aims to reduce stigma, strengthen social connections and help people affected by dementia remain active and valued members of their communities.
WHY OUR COMMUNITY WORK MATTERS
Creating Communities Where Everyone Belongs
Dementia affects individuals, families and entire communities. We believe everyone deserves understanding, dignity and the opportunity to remain connected. Through education, inclusive activities, community involvement and meaningful partnerships, Dementia Outreach CIC works to create welcoming environments where people living with dementia and their carers feel valued and supported.
